Who we are
This website is operated by Domiano Eye Care, an optometry practice with offices in Old Forge and Clarks Summit, Pennsylvania. This policy explains how information is handled when you use www.domianoeye.com.
This is a website privacy policy. It covers what happens on this site. Your clinical records and protected health information are governed separately by our HIPAA Notice of Privacy Practices, which is provided to you at the office and is available on request from either location.
What information we collect
We collect only what you choose to give us, plus basic technical data every website receives.
Information you provide
When you submit the appointment request form, we collect the details you enter:
- Your name, phone number, and email address
- Your preferred office, and (optionally) preferred date and time
- Anything you type in the optional “reason for visit” field
- Your confirmation that we may contact you about your request
Please don't include detailed medical information in the online form. A short note like “annual exam” or “dry eye” is helpful; a full medical history is not needed and is better discussed privately at your visit or by phone.
Information collected automatically
Like most websites, we use a privacy-focused analytics service (Vercel Analytics) to understand how the site is used in aggregate — for example, which pages are visited most and roughly how many people visit. This service does not use cookies to track you across other websites and does not build individual visitor profiles. Our web host also keeps standard server logs (such as IP address, browser type, and time of request) for security and reliability.
We do not use advertising trackers, do not sell your information, and do not share it with data brokers.
How we use it
- To respond to your appointment request — by phone or email — and schedule your visit
- To answer questions you send us
- To keep the website secure, reliable, and working well
- To understand, in aggregate, how the site is used so we can improve it
We use the information you give us only for these purposes. We do not use it for marketing lists, and we do not contact you about anything other than your request unless you ask us to.
Who receives it
Appointment requests are delivered directly to our office staff by email so we can call you back. To do that, the form uses a transactional email delivery service (Resend) to send the message, and the website itself is hosted on Vercel. These providers process the information only to deliver the service to us and are not permitted to use it for their own purposes.
We do not sell, rent, or trade your information. We may disclose it if required by law, or to protect the safety of a person or the security of the site.
How long we keep it
Appointment request emails are kept only as long as needed to schedule and follow up on your visit. Once your appointment is made, the relevant details become part of your patient record at the office, which is handled under our HIPAA Notice of Privacy Practices — not this website policy. Aggregate analytics data does not identify you and is retained per the analytics provider's standard schedule.
How we protect it
The site is served over an encrypted connection (HTTPS). Form submissions travel encrypted to our email delivery provider and on to our staff. We use a hidden anti-spam field and rate limiting to reduce automated abuse of the form. No method of transmission over the internet is completely secure, so if you would rather not use the online form, you are always welcome to call either office directly.
Your choices
- You never have to use the online form — every service is available by phone.
- You can ask us to delete an appointment request you submitted, or to correct information in it, by calling either office.
- Most browsers let you block or clear site data; the site will continue to work without it.
Children
We provide pediatric eye care, but this website is intended to be used by adults. Appointment requests for a child should be submitted by a parent or guardian. We do not knowingly collect information directly from children under 13 through this site.
Links to other sites
Some pages link to outside websites — for example, Google Maps for directions. Those sites have their own privacy practices, which we do not control.
Changes to this policy
If we change how the website handles your information, we will update this page and the effective date above.
